President - Mark Raderstorf, MA, LP, CCM, CRC - Owner, Founder and President
of BMI, Mark has over 25 years of experience in rehabilitation, with a strong foundation in working with individuals with
physical disabilities such as spinal cord injury, brain injury, and back injuries. Mark has successfully assisted numerous
clients with disabilities in return-to-work planning and reasonable accommodations. More recently, Mark has focused on
training and process development for behavioral health disability management. He has trained hundreds of claims and health
professionals on topics related to behavioral disability management and psychological issues in the workplace, as well as
publishing several articles in this area.

VP Workplace Interventions - Kate Harri, MA, LP - Kate Harri, MA, is a
Licensed Psychologist with over 30 years experience in the mental health field, specializing in workplace violence and critical
incidents. She has worked in probation, psychiatric, private practice and corporate settings. She has directed two Employee
Assistance Programs that specialize in providing consultation to managers in addition to supporting employees in resolving
issues impacting their health and productivity. Ms. Harri has authored several articles on mental health concerns in the business
environment. She conducts critical incident interventions in the workplace, risk
assessments concerning workplace harassment, hostility and violence. She also develops workplace violence crisis intervention
teams, provides threat assessments, career and management coaching.

VP Operations - Cathy Cato, BSN, MPH, COHN - Cathy joined the BMI staff in 2005
to serve as Quality Assurance Manager. She received her Bachelor of Science in Nursing and Master of Public Health, Occupational
Health Focus, at the University of Minnesota. She is a Certified Occupational Health Nurse Specialist and has 20 years of experience
in the field of occupational health and disability management. She has served in consulting and leadership roles at Ridgeview
Business Health Services, 3M Corporate Occupational Medicine, and most recently, Allina Hospitals and Clinics.

National Business Development Consultant - Michael Coupland, CPsych, CRC - Michael
is a Certified Psychologist, Certified Rehabilitation Counselor, and Certified Methods-Time Measurement Analyst. Mr. Coupland
founded the HealthServ, AssessMed & AssessAbility Disability Evaluations companies that have performed over 50,000 evaluations.
He is the developer of the AssessAbility Functional Medicine Evaluation system. Michael has served on the Return to Work Committee
of the International Association of Industrial Accident Boards and Commissions (IAIABC). He wrote the Virginia Commonwealth
University/Medical College of Virginia coursebook on Functional Capacity Evaluation and as an Adjunct Professor taught that course
throughout North America and in Hong Kong. Michael travels throughout North America engaging in Business Development from his
home in Palm Beach, FL.

Account Managers

Account Manager - Ali Keyes - Ali has been an Account Manager since
2004. She has a background as a licensed insurance agent in employee benefits and has been in the insurance industry for over 15 years.
She has two cats and raccoons, chipmunks and birds frequent her home.

Account Manager - Becky Reed - Becky has been an Account Manager with
BMI since 2003. Prior to working with BMI she was employed with HealthEast as a Managed Care Representative. She has her BA degree
from UW Stout in Psychology, where she also was the captain of the Women's Rugby Team. In her spare time she enjoys showing
horses, and working on her new house.

Account Manager - Kerri Booher - Kerri is new to BMI this year and is a great fit with the office.
She has a 12 year career background in training, recruiting, and operations management with a BA degree in Psychology.
Outside of work hours Kerri keeps herself busy by running, biking, skiing and volunteering for the St Paul Jaycees.

Clinician Network Manager

Clinician Network Manager - Phyliss Haugen, SPHR - Phyliss is the Clinician Network Manager
for BMI, responsible for recruitment and credentialing of new clinicians for our growing panel. She is certified as a Senior Professional
in Human Resources and has served in diverse aspects of vocational and career management in leadership, marketing, sales and consulting roles.
Her experience includes vocational rehabilitation consulting, corporate outplacement consulting, recruitment, and college career placement.

Operations

Office / IT Manager - Chandelle Mohr - Chandelle joined the BMI staff in
2007 in a new position, Office/IT Manager. She received her Bachelor degree in Computer Science at Bemidji State
University. She came to BMI with a wealth of experience in: Information Technology, including website development;
administrative positions; and disability claims experience. Chandelle is responsible for the day to day administrative
and IT operations at BMI, and works closely with the Account Managers and Quality Assurance staff to ensure producing
high quality reports. Much of her leisure time is spent with her family at their Canadian cabin at Rainy Lake (there is
even a computer there). She has a passion for the outdoors and animals, especially her golden retriever, Keetna.

Business Manager - Sandy Mattson - Sandy has been with BMI since 2000.
She has seen BMI grow "beyond our wildest dreams" these past years. Sandy is responsible for the accounts payable and receivable,
office equipment/supplies maintenance, purchasing. She enjoys spending time with family and friends, reading, and engaging
in church activities.

Editor / Trainer - Sharmaine Rich - Sharmaine began her career at BMI as an Account Manager
and in January '08, started in a new position - Editor / Trainer. She has an extensive background in law as a paralegal and legal investigator and
maintains a degree in Philosophy. She has a teenage son and is an avid reader and gardener. She still loves to
take beach vacations in her home state of North Carolina.

Transcription Manager - Sharon Jonas - Sharon came to BMI from a distinguished
career in Transcription and Administrative duties in the industries of insurance, corporate food franchise and civil engineering
where she was named Employee of the Quarter and received many Customer Service Awards. As Transcription Manager, Sharon has
been instrumental in shifting BMI’s dictation services from analog to digital, which allows reports to be dictated from virtually
any location. In addition to working for BMI, she also has her own digital transcription business. In her free time, Sharon
enjoys movies, reading and visiting with her sons on the West Coast and Midwest.

Administrative Assistant / Report Editor - Janet McCartney - Janet joined
the BMI family in August of 2006. Janet formerly held an Administrative Assistant position for a school district. Janet
started as a receptionist with BMI but has now transitioned into report processing and editing which she is finding very
interesting and challenging. She also assists with quality assurance staff and account managers. Outside of the office,
Janet and her husband enjoy the outdoors.

Transcription / Administrative Assistant - Robin Myers - Robin has worked as an
administrative assistant, transcriptionist, and word processor for over 25 years with experience in medical reports, reinsurance
documents, law enforcement reports/interviews, among other areas. In addition to working with BMI, she also is a private contractor.

|
Staff Support Specialist - Katelyn Beilke - Katelyn is a Staff Support Specialist
for interoffice coordination and administrative support to the President. She also assists clinicians and participates in the creation of
marketing materials. She is currently pursuing her degree in business and greatly enjoys the outdoors.

Receptionist / Administrative Assistant - Tanya Blome - Tanya joined BMI in
the fall of 2007 and is the first point of contact for walk-in customers and outside telephone calls to our offices. She has
over ten years of administrative and customer relations experience, which will be very apparent as you communicate with her.
Her additional responsibilities include assembling incoming files for the Account Managers and performing final edits for
outgoing reports. When not at BMI, Tanya loves to spend time with her family, and her dog, a Boston Terrier named Harley.
She has taken paralegal classes and enjoys reading true crime books and watching true crime televisions shows.

Clinicians

Psychological Quality Assurance Manager - Jennifer Kurtz, PsyD, LP - Dr. Kurtz received her
Doctorate degree from the Minnesota School of Professional Psychology. Her experience includes work for the Hennepin County Prepetition
Screening Program reviewing cases for civil commitment, and with the Anoka-Hennepin School District performing intellectual, social/emotional,
and functional evaluations for the Special Education Department. Dr. Kurtz's role at BMI is Quality Assurance Manager - Psych and she also
does disability case reviews, psychological testing, and case management services.

Quality Reviewer - Tyler Arvig, PsyD - Dr. Arvig is a graduate of the Minnesota
School of Professional Psychology. He has experience conducting cognitive, personality, and behavioral assessments. Dr. Arvig spent
two years at Wiger & Associates, developing skills in assessing functional impairment using clinical interviews and psychological
testing. He also has an academic interest in the relationship between physical impairment and psychological functioning. In 2006,
he published an article on this in Psychological Reports, a nationally recognized and peer reviewed journal. His primary role is
providing quality assurance services at BMI, but he also conducts test interpretation.

Jennifer Nash-Wright, Psy.D, L.P. - Dr. Nash-Wright has a doctorate in Clinical
Psychology from the Minnesota School of Professional Psychology, and Masters Degree in Counseling Psychology from St. Mary's University
of Minnesota. Dr. Nash-Wright has over 15 years of experience providing psychological services in a variety of clinical settings
including private practice. She has a competency in co-occurring mental health and substance abuse disorders. Dr. Nash-Wright's
primary role is performing disability case reviews.

Karen Kane, MD - Dr. Kane is a consulting physician at BMI where she performs
file reviews, MD to MD consultations on challenging disability cases. She has almost 20-years of clinical experience treating
musculoskeletal injuries, including back and neck pain, and repetitive stress injuries such as carpal tunnel syndrome. Prevention
and management of chronic pain disorders are among Dr. Kane's areas of interest and expertise. Dr. Kane completed residency and
fellowship training at the Mayo Graduate School of Medicine in 1985 and is Board Certified in Internal Medicine, Preventive
Medicine, and Occupational Medicine. Dr. Kane currently maintains an active practice at Physician's Neck and Back Clinic. Along
with her clinical work, Dr. Kane is a Clinical Instructor with the University of Minnesota and provides supervision and instruction
to Family Practice residents. Since 2003, Dr. Kane has functioned as one of BMI's Physician Advisors to General Electric
Disability Benefits Center. Karen is the Medical Director for BMI.

Dean Knudson, MD - Dr. Knudson has 20 years of clinical experience treating a
range of psychiatric disorders on both an inpatient and outpatient basis. Presently he is a practicing psychiatrist through the
Allina Behavioral Health Clinic at Abbot Northwestern Hospital in Minneapolis. Certified by the American Board of Psychiatry
and Neurology, Dr. Knudson has a special interest and expertise in Geriatric Psychiatry. Along with his clinical work, Dr.
Knudson is also an Associate Professor at the University of Minnesota School of Medicine where he supervises medical students.
He was chosen as the 2003 Minnesota Provider of the Year by the National Alliance for the Mentally Ill (NAMI). Dr. Knudson has
been a BMI Physician Advisors to the General Electric Disability Benefits Center since 2003. He is presently the Psychiatric
Medical Director for BMI.

helps us form a collaborative effort. We are able to work together with BMI to provide the best possible plan in assisting with
